Fictional worked example
From monthly rates to a benefit proposal you can check.
Follow two fictional medical plans through enrollment, a 75% employer contribution and employee payroll deductions. Download the source packet and the actual six-page proposal book exported from Planlined.
Proposal: 6 pages. Fictional source packet: 2 pages. Both download directly without signup.
Start with four tiers and ten employees
The source packet lists current monthly rates and a fictional renewal that is 10% higher in every tier. Enrollment stays at five employee-only, two spouse, one child and two family contracts. The case covers October 1, 2026 through September 30, 2027; the comparison annualizes both sets of rates over 12 months.
| Coverage tier | Enrolled | Current rate | Renewal rate | Employee / paycheck |
|---|---|---|---|---|
| Employee only | 5 | $500 | $550 | $63.46 |
| Employee + spouse | 2 | $1,000 | $1,100 | $126.92 |
| Employee + child(ren) | 1 | $900 | $990 | $114.23 |
| Family | 2 | $1,400 | $1,540 | $177.69 |
Check the premium and contribution math
Current monthly premium is (5 × $500) + (2 × $1,000) + (1 × $900) + (2 × $1,400) = $8,200. Renewal is $9,020. Annual premium moves from $98,400 to $108,240, an increase of $9,840, or 10%.
The employer pays 75% of each tier premium in both plans. At renewal, that is $6,765 per month for the employer and $2,255 for employees together. For employee-only coverage, $550 × 25% = $137.50 per month; $137.50 × 12 ÷ 26 pay periods = $63.46 per paycheck after rounding.
Page 4 of the proposal shows rates and enrollment. Page 5 shows payroll deductions and employer contributions. The group deduction total is calculated before rounding individual tier amounts; multiplying rounded paycheck amounts may differ by a few cents.

How this sample was made
Verified October 16, 2026 in the existing Planlined application. The initial file import and one AI retry failed before producing plans. We then entered the reviewed fictional values manually through the normal product screens, saved the case to the cloud, reopened it and checked the rates, benefits, enrollment and contribution totals.
The six-page book was generated by Planlined’s proposal-book export and saved in its Document Library. It includes the cover, overview, financials, medical rates, deductions and census. The optional benefit-detail page had a page-fit issue and was omitted; projections and client acceptance pages were also omitted. This example demonstrates the reviewed cost comparison and export workflow, not successful automated extraction or a complete benefit guide.
The export retains its “Source evidence unverified” caveat. That means this manually entered example does not demonstrate verified source-page links. The only change to the downloaded PDF was replacing a personal organization name in its Author metadata; page content was unchanged. The product’s recommendation label is illustrative and does not recommend a real insurance plan. Its Overview retains the default “New Business” proposal-type label; the input rates are labeled current and renewal.
For the complete inputs, download the fictional values and expected totals (JSON). Use the free comparison template to work through your own documents, or check a contribution assumption with the payroll deduction calculator.
